West Branch Road, Prudenville, MI 48651, for the item: DESCRIPTION This work consists of providing all labor, materials and equipment to install freeway delineator and maintain traffic as specified herein, for both I-75 and US-127 and all freeway ramps within the counties of Crawford, Ogemaw and Roscommon. Work shall include the installation of all median shoulder delineators (yellow sheeting) located on I-75 and US-127, and all median shoulder (yellow sheeting), outside shoulder (white sheeting) and wrong way (red sheeting) on all freeway ramps along I-75 and US-127. Installation of delineators shall include a single 3 inch by 12 inch delineator per the Special Provision Plan for Sheeted Rectangular Delineator Reflector and spaced according to MDOT Standard Plan R-127. The Board may not adopt its proposed 2016-17 budget until after the public hearing. Request for Proposals Background and History The AuSable Free Clinic Inc. (AFC) is requesting proposals to help us implement new free clinic coordinator position to allow us to continue to provide free health services to the underserved populations in our community. This free clinic coordinator will be responsible for facilitating the continued provision of clinical care and the use of the free clinic resources including; community outreach and collaboration with other health and human service agencies, oversight of utilization of AFC financial resources, and coordinating care for the vulnerable populations in our community. Project Goals We are requesting proposals from community agencies or individuals to help us continue our mission. The goals of this project will be: •To continue our care coordination efforts with local agencies to help navigate the clients we serve through the healthcare and human service setting. •To continue to improve our outreach efforts to reach those in need throughout our rural community through networking, advertising and collaboration. •To continue our mobile clinic efforts by utilizing our volunteer base to provide care to those in need in their community of residence. •To manage AFC resources including volunteer supervision/recruitment, and finances including expense and revenue oversight and grant writing where able. •To report to the AFC Board of Directors and assist in coordinating board meetings with the board Secretary. •To continue AFC efforts to strengthen interagency collaboration and integration of services to better utilize the often scarce resources available in the rural setting. Timeline, Available Funds, Proposal Format Proposals should be submitted to the AFC Board of Directors in writing by June 30, 2016. We will be having informational meetings June 17, 2016 AND June 24, 2016 at the Department of Health and Human Services offices in Grayling at 11:00 am.
